How to fill the form

1. Print the downloaded PDF on A4 paper.
2. Fill the student’s details: full name (CAPITAL letters), date of birth, age (as on 31st March), blood group and category.
3. Fill parent/guardian details: names, occupation, contact numbers, residential & permanent addresses and annual income (if requested).
4. Provide previous school details: name, class last attended, TC status and last result/percentage.
5. Attach passport-size photos and required documents (see checklist below).
6. Sign the declaration at the bottom of the form (parent/guardian signature).
7. Submit the completed form and documents at the school office during working hours.
